### Job Duties • Is familiar with the Consumer Directed Community Supports (CDCS) service, person-centered planning, and the process of developing a support plan. • Develops individualized support plans tailored to the unique needs of each individual. • Maintains accurate documentation and data collection related to consumer progress and service delivery. • Ensures quality and updated program services. • Provides outreach in the local service area. • Is familiar with community resources and support services. • Some local travel required. ### Minimum Education Required • Minimum 2-year degree in human services or disability related fields.
