Work where you matter.** **A Brief Overview** Interprets and implements management policies and operating practices to support patient care navigation in the clinic setting. The Coordinator's primary responsibility is to manage care for patients of the primary care practice to promote effective education, self-management support and timely health care delivery. The position does include some patient triage. The RN will work with the clinic leadership and Medical Director of the practice to develop this position to best serve the needs of the patient population and the primary care teams. **What you will do** + Develops a workflow to ensure smooth transition of care for patients treated in a facility (inpatient or emergency department), by specialty physicians, or by another health care provider. + Involves patients in activities to improve their health (patient engagement) and educates patients about self-management tasks they can undertake to gain greater control of their health status. + Works with social services and nursing to identify, plan for, and provide an appropriate follow-up plan of care for patients. Communicates this plan of care to patients’ families, medical providers, hospital staff when appropriate, community support, other agencies and departments. + Collaborates with physicians, providers, and practice staff in identifying appropriate patients for care management and developing policies for patient selection. + Serves as a primary resource for the clinic care team on education of changing payer requirements. + Provides follow-up contact with patient as indicated to ensure compliance with recommendations – medications, lab/x-ray, specialists visits, PCP visits, dieticians, CDE, etc. + Anticipates the needs of the patient population, seeing that necessary documentation and pre-visit planning is completed and requested before patient visit. + Works with patient and patient’s care team to coordinate change readiness, needs assessment and develop an individualized treatment care plan. + Collaborates with patient, provider, and other care team members in assessing the patient’s progress toward individual health care goals. + Assesses barriers when patient has not met treatment goals, is not following treatment plan of care, or has not kept important appointments, and uses judgment to develop plan for overcoming barriers. + Responsibilities include interviewing, hiring, developing, training, and retaining employees; planning, assigning, and leading work; appraising performance; rewarding and coaching employees; addressing complaints and resolving problems. **Essential Qualifications** The individual must be able to work the hours specified.
